Payment Options: Full payment required at time of services. Half down payment required when ordering any materials. Payment plan available upon approval.
Insurance: We accept assigment on all insurance companies covered by our office. Only non-covered charges or copayments are due at the time of service. When we here from the insurance company, we will be you any additional non-covered charges.
Warranties: A shipping and handling charge is applicable to all warranty work. There is no charge for frame warranties. All frames are warrantied for one year from breakage during normal wear.
Multiple Purchases: Order a second pair of glasses for the same patient within 90 days, and receive 25% off the lesser priced pair of glasses.
